Who Must File

Before participating as a service provider in one of the four federal universal service programs, a company must obtain a Service Provider Identification Number (SPIN) by filing an FCC Form 498, Service Provider Identification Number and General Contact Information Form. All providers and recipients must review the instructions prior to completing the form. The instructions for completing FCC Form 498 are available in PDF format.

Information provided on this form enables the service provider to receive program disbursements. The FCC Form 498 allows service providers to use the same information for one or more of the programs or different information for each program.

First-time applicants will be assigned a SPIN by USAC. The FCC Form 498 collects the following information: service provider name, address, phone numbers, email addresses, contact names, and billing and collection information.

Service providers who have previously filed an FCC Form 498 and obtained a SPIN must file a revision to the FCC Form 498 when any of the following occur:
  • Change to contact persons, for programs, remittance, General Contact or Company Officer
  • Change of address for any of the programs, or to the General Contact or Company Officer
  • Changes to the banking and remittance information on file
  • Changes to their FCC Form 499 Filer ID, such as deactivation, consolidation or merger
  • Change how remittance statements are received
  • Changes to TAX ID, business structure, or business type
  • To deactivate a SPIN
  • To notify USAC of a sale, merger, or consolidation of one SPIN into another
