Filing & Managing My 498

Revise a SPIN

Service providers may revise payment and contact information by revising the FCC Form 498 (Service Provider Identification Number and General Contact Information Form) and submitting the form to USAC. USAC strongly encourages online filing and e-certification of this form in order to more quickly process the request.

To begin the online filing process, please visit USAC's E-File page. To download a blank form or view the form instructions, please visit our forms page.

Service providers must indicate, by checking the appropriate box, whether the form is an original application or a revision. To initiate revisions, a completed FCC Form 498 must be submitted to USAC. The form must be signed by a Company Officer as listed on the previously filed FCC Form 498 or the new Company Officer if that person has changed.

It is critical for the service provider to keep the information on this form current as it may affect the timeliness of payment.

Please review the Service Provider FAQs and Forms Processing sections of the USAC website for more information regarding the FCC Form 498 process.

Where to File

First-time service providers can file FCC Form 498 using USAC's E-File system, but you must send an original, Officer-signed copy to USAC the first time. After the first filing, the Company Officer will be able to certify revisions online via the E-File system.

Please send the original, signed form to:

Universal Service Administrative Company
Billing and Disbursement
Attn: FCC Form 498
2000 L Street NW, Suite 200
Washington, DC 20036

Revisions may also be emailed or faxed.
Email: Customer Operations ATTN: FCC Form 498
Fax: (888) 637-6226 ATTN: FCC Form 498

What to Expect After Filing

If you submitted your FCC Form 498 using E-File, you will receive a confirmation email notifying you that USAC received your submission. If you do not receive this email, check your spam and/or log back into E-File to verify that your form was submitted.

If you mailed in your paper FCC Form 498 to USAC and did not use E-File, you will not receive a confirmation of receipt. If USAC is unable to process your request due to missing documentation or information, USAC will email the General Contact listed on the form, explain the issue, and request resubmission.

For both paper forms and forms filed electronically, if all appropriate documentation and information is received by USAC, it takes approximately two weeks to review, process, and send out your new SPIN. After one week, you may contact USAC to find out the status of your SPIN application (FCC Form 498) by calling (888) 641-8722.
