Filing & Managing My 498

FCC Form 498 (Service Provider Identification Number and General Contact Information Form) is used to collect contact, remittance, and payment information for service providers that receive support from the federal universal service programs - High Cost, Lifeline, Rural Health Care, and Schools and Libraries - administered by USAC.

All service providers participating in a universal service program must use the current FCC Form 498 when submitting for a new Service Provider Identification Number (SPIN) or revising existing information about the SPIN.

Key Elements

  • Only the Company Officer can certify a new or revised FCC Form 498
  • All service providers must provide electronic banking information
  • Service providers must provide their Dunn and Bradstreet (DUNS) Numbers
  • The four uses for the FCC Form 498 appear together at the beginning of the form
Once USAC receives an FCC Form 498, USAC will begin its review process.
