Step 8: File Annual Certification
Service providers must file the Service Provider Annual Certification Form (Form 473) each year that they provide services under the program.
Service providers certify each year on the Form 473 that they will comply with program rules.
The service provider certifies that the invoices it submits to USAC will be based on bills already submitted to customers (applicants) and will exclude any charges already invoiced to USAC. Additionally, the service provider certifies that it acknowledges USAC's authority to request additional documentation to support any invoices for a period of five years following the submission of the invoices.
The certifications on the Form 473 relates to the entire funding year. USAC will not pay invoices - the Service Provider Invoice Form (Form 474) submitted by the service provider or the Billed Entity Applicant Reimbursement Form (Form 472) submitted by the applicant - unless USAC has a Form 473 on file for the funding year featured on the invoice.
