Quarterly Reports

Paragraph 126 of the Selection Order requires projects to submit quarterly reports to USAC and the FCC. All reports are filed in the Rural Health Care docket.

Filing Quarterly Reports

To file your quarterly report with USAC, please upload the document via SharePoint. To file your report with the FCC, please upload the document via the Electronic Comment Filing System (ECFS).

USAC recommends that projects follow the template shown below when submitting the quarterly report to ensure that all quarterly reports are filed in a consistent manner.

Should be the Rural Health Care docket, which is 02-60

Name of Filer

The name of your project, rather than the name of the contact person

Type of Filing

Select report from the drop-down menu

File Name

Recommend naming the file “Quarterly Report” and then the month and year the report is due, for example “Quarterly Report October 2010.”
