Duplicate Address Resolution for Vermont

During the initial NLAD loading process in Vermont, subscribers who reside at the same address as another Lifeline subscriber must undergo duplicate address resolution because they are identified as potential household duplicates. Only one Lifeline Program benefit is allowed per household (See Lifeline Program rules). Follow the instructions below to resolve duplicate address records.

Step 1: Identify the duplicate address records

    1. Log in to NLAD.
    2. Under the Reports and Tools section, select Reports.
    3. Select Detail Duplicate Subscriber Report.
    4. Select the study area code (SAC).
    5. Select your preferred report format (either "display" or "CSV").
    6. Click Submit.

Subscribers identified as household duplicates will appear as "Duplicate Address" in the "Duplicate Type" field of this report.

Step 2: Send a One-Per-Household Worksheet

Service providers should use the Lifeline Household Worksheet  (Spanish Version coming soon) - also known as the Independent Economic Household (IEH) Worksheet - to determine if the household is eligible. When providing household worksheets to subscribers, the communication must comply with the requirements of Section 54.405(e)(1) – de-enrollment generally – of the Lifeline Program Rules.

If the household is eligible: If the subscriber completed a one-per-household worksheet is found to be eligible, follow Steps 3 and 4 below to de-enroll and re-enroll the subscriber in NLAD.

If the household is NOT eligible: If the subscriber did not complete a household worksheet, or if the household is not eligible, complete Step 3 below to de-enroll the subscriber record from NLAD.

Keep copies of the completed household worksheets in accordance with the Lifeline Program’s documentation retention rules (47 C.F.R. Section 54.417(a)).

Step 3: De-Enroll the subscriber from NLAD

Service providers will have until January 26, 2018 to resolve any duplicate address records found on their "Detail Duplicate Subscriber Report."

To de-enroll a subscriber from NLAD:

    1. Log into NLAD and go to the De-Enroll Subscriber section.
    2. Enter in the subscriber's Lifeline Program phone number.
    3. Select Duplicate Resolution as the "De-Enroll Type" from the drop-down menu.
    4. Select the De-Enroll Reason (De-Enroll Leaving) and enter in the De-Enroll Date.

Step 4: Re-Enroll the subscriber in NLAD (if applicable)

If the household is eligible, service providers are not required to gather new certification forms or examine proof of eligibility. For this reason, service providers can keep the same service initiation date listed for the subscriber that was entered during the NLAD loading process.

To re-enroll the subscriber, follow these steps:

    1. Log into NLAD and go to the Enroll Subscriber section.
    2. Enter all of the required subscriber information.
    3. In the Subscriber Eligibility Information section, select Yes for the "Independent Economic Household" flag and enter in the IEH Certification Date.

Note: If a subscriber fails the Third Party Identify Verification (TPIV) check during the re-enrollment process, service providers can resolve the error by requesting documentation from the subscriber and entering a “dispute resolution” request for TPIV errors. USAC De-Enrollment from NLAD

On February 2, 2018, USAC will de-enroll any remaining subscribers from the "Detail Duplicate Subscriber Report" who have not been de-enrolled in accordance with the guidance provided above. As of this date, service providers should also no longer include these subscribers on their FCC Forms 497 for reimbursement.

To locate the "Duplicate Resolution De-Enroll Report" in NLAD, follow these instructions:

    1. Under the Reports and Tools section, select Reports.
    2. Select Duplicate Resolution De-Enroll Report.
    3. Select the study area code (SAC).
    4. Select your preferred report format (either "display" or "CSV").
    5. Click Submit.
