Calendar / Reminders

  • March 31, 2008 - Eligible telecommunications carriers (ETCs) must begin using bill inserts or language on a monthly bill to notify their Lifeline and Link Up customers about the DTV transition. For more information please see the March 19 DTV Important Notice
  • AUGUST 31, 2008 - Eligible telecommunications carriers (ETCs) must submit all original 2008 Lifeline Verifications and 2008 Low Income Certifications (non-federal default states) to USAC no later than August 31, 2008.
  • OCTOBER 31, 2008 - Eligible telecommunications carriers (ETCs) must submit all follow-up 2008 Lifeline Verifications to USAC no later than October 31, 2008.
  • MARCH 31, 2009 - Eligible telecommunications carriers (ETCs) must submit all original and revised FCC Form 497s to USAC for 2007 data months no later than March 31, 2009.

~ REMINDERS ~

  • One Form 497 must be submitted for each month.

 

  • All Form 497s must be signed, dated, and certified on page 2.
  • Please submit each Form 497 once, unless you are making revisions to a previously submitted form.
