Revise your Service Provider ID (Form 498)
Information on how to revise a Service Provider Identification Number (SPIN).
Service providers may revise payment and contact information by amending the Service Provider Identification and Contact Information (Form 498) and submitting the form to USAC. USAC strongly encourages online filing and e-certification of this form in order to more quickly process the request

Service providers should indicate, by checking the appropriate box, whether the form is an original application or a revision.
To initiate revisions, a completed FCC Form 498 must be submitted to USAC. The form must be signed by a Company Officer as listed on the previously filed Form 498 or the new Company Officer if that person has changed.
It is critical for the service provider to keep the information on this form current as it may affect the timeliness of payment.
