How To: File the 499A online!
Filing your FCC Form 499-A is easy and convenient!
Log Into The Form 499 Data Collection System:
- Go to http://form499.universalservice.org
- Click on the "LOGIN" button
- Enter your Logon User ID and Password and then select the "Login" button. You may receive a Security Alert box indicating that you are about to be redirected to a connection that is not secure. Please select "Yes" to continue.
NOTE: Encrypted data is designated by a lock in the lower right hand corner of the screen. Entering and transmitting the User ID and Password is done using a secure connection (https). Once the information is validated, the standard connection (http) is used and the warning message is displayed. When a form is selected, the connection is switched back to the secure mode. A user can manually change the "http://form…" to "https://form…" and the entire session will be transmitted securely.
- After a successful login, you will arrive at the default "Search Forms" screen.
Creating An April 2007 499-A For The First Time:
- Select the "Create New Form" menu option (first option on top left of the screen)
- Confirm that "499A for April, 2007" appears in the drop down box next to "Filing:" If it is not, click on the down arrow and select "499A for April, 2007". Type in the Filer 499 ID that you wish to create an April 499-A filing for. Select the "Submit" button.
NOTE: Your Filer 499 ID will begin with an "8" and is six numbers long.
- An April 2007 499-A will be opened using Adobe Acrobat. Please be patient as the form opens, the 499-A is seven pages long.
Entering Data Into The Form 499-A:
- When a form is created, the system automatically copies certain fields using previously submitted form(s). Please review pre-populated data and re-confirm. You may change pre-populated data by simply selecting the field and overwriting with the correct information.
NOTE: You cannot change Line 102 "Legal name of reported entity", Line 103 "IRS employer identification number" and Line 104 "Name telecommunications service provider is doing business as" online. These types of changes require submission of legal documentation to the Form 499 Data Collection Agent.
- For any field requiring new information, use your mouse to select the field and then type in the new data.
To Save The Form 499-A As A Work In Progress:
- You will see a bright green " SAVE AND CLOSE" button at the top right of each page of the Form 499-A. In addition a "SAVE" button appears at the bottom of page 7. By selecting any of these "SAVE" buttons, you are directing the online system to save your form in its incomplete state. After selecting the "SAVE AND CLOSE" button, you are re-directed to a screen stating "You have saved your 499 submission as a work in progress. Please note that you did not submit your data to the Form 499 Data Collection Agent (DCA)."
- In order to return to your incomplete 499-A, select the "Search Forms" menu item at the left of the screen. Type in your Filer 499 ID and select the "Search" button. A table will appear listing all forms available for viewing online. Under the column labeled "Form" find the row labeled " 499A - April 2007 Filing." You may open this incomplete form by clicking your mouse on the "499A - April 2007 Filing" label. The form opens using Adobe Acrobat. Continue to work on your 499-A.
To Submit Your 499-A To The Data Collection Agent As Complete:
- If the 499-A is complete, and all information is accurate to the best of your knowledge, select the "SUBMIT" button at the bottom of page 7.
- After selecting the "SUBMIT" button, you are re-directed to a screen with the message "Thank you for submitting your 499 worksheet to the Form 499 Data Collection Agent (DCA)! Your Confirmation Number is XXXXXXX.."
NOTE: Each 499-A submission will receive a unique confirmation number. - Please note that your submission will not be considered complete until you print your submission, provide an officer's signature, and mail the signed form to:

To Revise A Previously Submitted 499-A:
- Select the "Search Forms" menu item at the left of the screen. Type in your Filer 499 ID and select the "Search" button. A table will appear listing all forms available for viewing online. Under the column labeled "Form" find the row labeled " 499A - April 2007 Filing." Confirm that the row for the column labeled "Status" is either "Certified" or "DCA Approved" and that the "Revise" button is not shaded. If all these conditions are met, you may select the "Revise" button and a new April 2007 499-A will be opened in Adobe Acrobat. If any of these conditions are not met, the April 2007 499-A cannot be revised at this time. Please contact the 499 Data Collection Agent at 888-641-8722 or email.
NOTE: If the "Filing Period" column states "Open For Revision", you are within the revision time frame to revise the form. If it states "Closed", the revision filing period is closed and you must mail your revision to Form 499 Data Collection Agent, Attn: USAC Customer Service, 2000 L St NW, Suite 200, Washington, DC 20036
Continue to re-confirm pre-populated data and/or enter new information in the same manner as your original submission (see Entering Data for the First Time). On page 7, if revenue was revised, please select the "Revised Filing with Updated Revenue Data" box. If only registration data was changed, please select the "Revised Filing With Updated Registration Data".
- You may save your data as a work in progress (see "To save the 499-A as a Work in Progress") or submit your revision to the DCA (see "To Submit your 499-A to the Data Collection Agent as Complete").
NOTE: Only those companies that have previously filed the FCC Form 499 and have received a Filer 499 ID can register and file online at this time. If you are filing the FCC Form 499 for the first time, please go to the USAC Forms page to download the form.
